Google Forms - Adding Images to your Questions

Adding images to a form can be useful in several ways. Maybe your form is a math quiz having students identify the correct graph.  Maybe it is having students identify particular body parts.  Or maybe you are working with non-readers so are creating an image-based form.  Regardless of why, it is very easy to add images into your form.

This post has how to add images to your questions.  Here is the post on how to add images to your form.  If you prefer a short video, here is one that shows How to Add a Picture to Your Form Questions.

To insert an image into a question title:

Click on the question that you want the image to appear BELOW.  Then click on the image icon in that appears on the right-hand side-

The Insert Image window will pop-up (similar to what it is with other Google Tools).  You have 6 options:
  1. Upload - an image from your device
  2. Take a snapshot - using the web cam
  3. By URL - paste in the URL of the picture
  4. Your albums - from your Google Albums (if pictures are stored there)
  5. Google Drive - from a folder in your G Drive
  6. Search (more on this one below)

If you click on search, note that it is only searching for images that have a creative commons license which allow for reuse -

If you use the search function, you can search by:
  1. keyword
  2. file type
  3. color scheme
When you select an image from a search, it displays the image's license at the bottom of the window.

Whichever method you use for selecting/finding your image, once you click Select it will place it beneath the question title.  There is a "more options" menu (three stacked dots) that will appear on the left side of the image -

You can decide on the  (1) image alignment, (2) change the image, (3) remove the image, and/or (4) add a caption to the image from this menu -

If you select to add a caption, it appears underneath the image -

When looking at the live form, this is how a caption appears -

You can also add am image to a question answer option.  When you hover over the possible answer, the image icon appears to the right -

Clicking it will bring up the same Insert image window, with the same options as above.  When you insert an image for an answer, while in edit mode it will appear underneath the answer -
When you are looking at the live form, it will try to arrange the possible answers to maximize the use of space -
